This is to certify that I , Mr.ANWAR RAM JAT have been offered the job of ( Rank ) :-- CHIEF COOK
on behalf of the owners of the vessel , MV :--SINCERE PRIDE. I hereby accept the offer as mentioned above .
 
I acknowledge , that I have been briefed on the following points , as mentioned below .
 
I accept that I shall be able to complete all the paper works required by the office , which have to be done by me on board .
 
I have understood the following as mentioned below & I accept the same .
1 ) Vessel Details :
a ) Name of the Vessel :MV SINCERE PRIDE
b ) Total No. of Crew & Officers on board :
c ) Total no. of Catering staff on board : 2
d ) Nationality of Officers & Crew on board : INDIAN & BANGLADESHI
e ) Cuisine to be served on board : INDIAN & BANGLADESHI
f ) Present Trading Pattern of the vessel / Best ports for maximum uplift of provision :
g ) Fixed working Hour ( Per Day & Saturday ) : hrs / day .
h ) Basic Salary : USD / Month . AS PER CONTRACT
i ) Over Time rate per day ( Sunday & Public Holiday Excluded ) : USD / hrs .
j ) I agree that I will make ( quality products ) of the follwoing on board & they need not be supplied by head Office :
  • Fresh Bread / Cakes etc.
  • Different Soups .
2 ) Job Specification :
I have been explained the importance of the below mentioned instructions & shall abide by them .
a ) Prepare Healthy / Clean & cost effective ( ie. within Budgets ) food for the entire Crew / Officers & Guests ( if any ) , everyday .
b ) Make correct Indent for supplies to be effected by the shore office indicating the present ROB on board for all items at that time .
c ) Do not incur any wastage of food either while Pre- Preparation or while Preparation . Cook only sufficient & adequate food needed for the no. of crew & officers on board .
d ) Month End Papers :--
  • Month End Inventory : Conduct a physical Inventory of each & every item . Weight & Count each & every item remaining at the end of each month & account the same in the Month End Forms correctly . All details must be filled as required in form .
  • Meal Reports ( ESM a/c , Owners a/c , Charterer’s a/c ) : All three reports are filled properly with all details as required in the form .
  • While taking over or Handing over from or to a new cook , must take proper Inventory for the take over / hand over respectively .
e ) Sanitation : Must maintain a clean & Hygienic Kitchen & other working areas .
f ) Store Rooms : All store rooms ( Fish / Meat / Dry / Veg etc. ) must be well arranged & all the provision must be stored properly . ( Certain thumb rules like Red Meats must be always stored in the lowest rack , white meat always on the top racks must be followed ) .
 
I accept that all above instructions have been well understood by me & I will perform as per the above norms , if the company incurres any loss due to my mistakes or incompetency , I shall bear the cost involved & the company can deduct that amount from my wages .
